MAKE A REFERRAL
Let’s plan the right support together
If you are referring someone for care or support, we are here to listen, understand the situation, and explain the next step. Our aim is to make the referral process clear, respectful, and straightforward.
HOW IT WORKS
A clear route from referral to support
A referral is the beginning of a conversation. We take time to understand the person’s needs, preferences, circumstances, and goals before discussing the most appropriate support options.
Who can refer
Families and representatives
You can contact us if you are supporting a relative or helping someone explore care for the first time.
Health and social-care professionals
Professionals can speak with us about a person’s needs, current circumstances, and possible support arrangements.
Organisations and referral partners
Local services, community organisations, and other partners can contact the team to discuss a referral route.
BEFORE YOU CONTACT US
The details that help us understand the situation
You do not need to have every answer before making contact. If available, it is helpful to share the person’s main support needs, preferred living arrangement, important routines, communication needs, risks or urgent concerns, current services, and the best way to contact the person or their representative.
- The person’s main support needs
- Their preferences and everyday routines
- Any important risks or immediate concerns
- Current support or professional involvement
- The best contact details and preferred communication method
How support begins
Step 1 — Contact
Send an enquiry or speak with the team about the person’s situation and the support you are exploring.
Step 2 — Understand
We ask relevant questions, listen to preferences, and explain which information or assessments may be needed.
Step 3 — Agree the next step
Where appropriate, we discuss the available options and explain what happens next. Any arrangement must be confirmed through the organisation’s approved process.
A RESPECTFUL PROCESS
Referrals should feel clear, not complicated
We know that asking for support can feel like a big step. We will take time to listen, answer questions, explain the process, and communicate clearly with the person, their representative, and relevant professionals.
- Clear communication
- Respect for the person’s choices
- A considered next step
